Without insurance, the Wegovy (semaglutide) pill costs $149 to $299 a month, depending on the dose. The Wegovy pen costs $349 a month at any dose from 0.25 mg to 2.4 mg, and Wegovy HD (7.2 mg) costs $399 a month. These are Novo Nordisk's self-pay prices, through NovoCare Pharmacy or the Wegovy savings card. New patients pay $199 for each of their first 2 monthly fills of the 0.25 mg and 0.5 mg pens, through December 31, 2026. The list price is $1,349.02 a month. Self-pay prices checked October 6, 2026.
If you're paying for Wegovy yourself, the price depends on which form you take and which dose.
Novo Nordisk's rule: "To pay these prices, you must fill your prescription using NovoCare Pharmacy or use the savings offer." The savings offer, which most people call the Wegovy savings card, works at your own pharmacy.

Wegovy pen and Wegovy HD cost per month
The pen is one price at every dose from 0.25 mg to 2.4 mg: $349 a month. Wegovy HD, the 7.2 mg pen, is $399 a month. Wegovy HD is for people who have taken 2.4 mg for at least 4 weeks, when their prescriber decides more weight loss is needed.

The $199 new-patient offer
If you're new to the Wegovy Savings Offer or NovoCare Pharmacy, your first 2 monthly fills of the 0.25 mg and 0.5 mg pens cost $199 each. This runs through December 31, 2026. After those 2 fills, it's $349 a month for any pen dose from 0.25 mg to 2.4 mg. NovoCare says pricing will be updated after December 31, 2026, so if your first fill falls in 2027, check the price first.
What Wegovy costs for a year
Pens come in 28-day months, so 13 fills cover a year (364 days). The pill comes in 30-tablet bottles, so 12 bottles cover 360 days.

These sums assume prices stay the same all year. Your first year starts at a low dose and steps up, and your prescriber decides your dose.
Wegovy list price, and the January 2027 cut
Wegovy's list price is $1,349.02 a month for the pill, the pen and Wegovy HD. It's the price before any discount, and it can still matter if you have insurance.
From January 1, 2027, Novo Nordisk cuts the list price of Wegovy (the 2.4 mg injection and the 25 mg pill) to $675 a month. Novo says this doesn't change its self-pay prices. It mainly helps insured people whose share is based on the list price, such as people with high deductibles or coinsurance.
Multi-month plans and TrumpRx
From March 31, 2026, Novo began offering multi-month self-pay plans through some telehealth providers. For the Wegovy injection, it was $329 a month on a 3-month plan, down to $249 a month on a 12-month plan. For the pill, it was $289 down to $249. Prices may have changed since, so check the current price and terms.
Hims & Hers began selling branded Wegovy, the pen and the pill, on March 26, 2026. You pay Novo's prices plus a membership: "$39 for the first month, $149 each month after."
TrumpRx.gov, a government site, lists the same self-pay prices and gives a coupon for Novo medicines at retail pharmacies.
What Wegovy costs with insurance
If your commercial (non-government) insurance covers Wegovy, Novo's savings card can bring your copay down to as little as $25 a month. It takes off up to $100 a month, for the pill, the pen or Wegovy HD. Our Wegovy savings card guide covers who qualifies and how the cap works.
Coverage depends a lot on your employer. In KFF's 2025 survey of employers, 16% of firms with 200 to 999 workers covered GLP-1s for weight loss. It was 30% at firms with 1,000 to 4,999 workers, and 43% at firms with 5,000 or more. Plans that cover them usually want your prescriber to get approval first, called prior authorization. Some also want you to try another treatment first, called step therapy.
If you're insured but pay the self-pay price, NovoCare says that money "will not count toward your deductible or out-of-pocket requirements and cannot be submitted for reimbursement to your insurer."
Medicare and Medicaid
The Medicare GLP-1 Bridge runs from July 1, 2026 to December 31, 2027. If you have Part D and qualify, you pay a flat $50 a month for Wegovy: the pill, the pen or Wegovy HD. You need to be 18 or older, prescribed it for weight management and not already getting a GLP-1 through your Part D plan. You also need a BMI of 35 or more, or a BMI of at least 27 or 30 with certain health conditions (your BMI when you started a GLP-1 counts). Our Wegovy savings card guide lists them. Your prescriber sends a request for approval, called a prior authorization.
If you take Wegovy for its approved heart use, or you have MASH (a type of fatty liver disease), type 2 diabetes or moderate-to-severe sleep apnea, you can't use the Bridge. You go through your normal Part D plan instead.
If you're on Medicare, Medicaid, TRICARE, VA or another government plan, you can't get Wegovy's $25 copay. Novo says you can still use its savings card to pay the self-pay price, outside your insurance.
Medicaid coverage is state by state. As of January 2026, 13 state Medicaid programs covered GLP-1s for obesity, and several states have cut coverage since, according to KFF. Check with your state's program.
Paying with an HSA or FSA
A GLP-1 prescribed for diagnosed obesity is generally eligible for your health savings account (HSA) or flexible spending account (FSA). IRS Publication 502 counts amounts you pay to lose weight as a medical expense "if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease)". Prescribed drugs count too. Your plan may ask for a letter of medical necessity. Medicines imported from other countries generally aren't eligible.
Where to fill your prescription
The Wegovy pill and Wegovy HD are stocked in about 70,000 US pharmacies, including CVS and Costco. To pay the self-pay price there, take the savings card to the pharmacist. With no insurance and a valid prescription, you can sign up by texting SAVE to 83757.
Cheap compounded copies
Compounded semaglutide is made by a pharmacy, not by Novo Nordisk, and it isn't FDA-approved: the FDA doesn't review compounded drugs for safety, effectiveness or quality before they're sold. The FDA declared the semaglutide shortage over on February 21, 2025, so mass-made compounded copies are no longer lawful. The FDA advises filling your prescription at a state-licensed pharmacy. Be especially wary of semaglutide sold for less than Novo Nordisk's own self-pay prices. Our US GLP-1 price page has more on where they stand.
